Immunization Records
To promote wellness and achieve optimal prevention of common infectious disease in the school community, Concordia has a Mandatory Vaccination Policy, requiring parents to submit documentation of the age-appropriate immunizations for each student before the student may attend classes.
- All NEW Students
- must submit a copy of all Immunization Records.
- All RETURNING Students
- Must submit a copy of Immunization Records if there there have been any updates since the last submission.
- If a returning student has not had any new immunizations during the year, the parent does not need to submit records.
You may submit immunization data to Concordia via one of the following ways. Prefer to receive the record before July 31.
- Submit a scanned photocopy of Immunization Records, with any updates, as part of the Annual Medical History and Permissions online via the website.
- Email a scanned copy of Immunization Records, with any updates, to health.office@concordiashanghai.org
Find below details on the required immunizations:
- Measles, Mumps, Rubella (MMR)
- 1 dose after age 12 months
- 1 booster vaccination at age 4-6 years
- Note: 1 dose of Measles & 1 dose of MMR is not considered complete
- Diphtheria, Tetanus, Pertussis (DTaP/DTP/DPT/Tdap)
- 5 doses by age 4-6
- Tdap/DT/Td booster vaccination at age 11-12 years
- Polio (IPV/OPV)
- 4 doses by age 4-6
- Varicella (Chickenpox) vaccine is highly recommended.
