Concordia requires parents to submit two medical forms for each student. Please note that a student will not be allowed to attend classes without these health forms on file in the Health Office.
Form 1: Student Physical Examination Form - Completed by Doctor
Concordia requires parents to submit a doctor-completed physical examination form for each student at the time of admission (the examination must be made within the 6 months immediately before the first day the student attends class) and repeat every three school years. Please note that a tuberculosis screening is required as part of the physical exam.
If you need to submit the form:
- Download the Student Physical Examination Form
- Print out the form
- Have a medical doctor fill in the form
- Submit the form to Concordia via the website. Prefer to receive the form before July 31.

Form 2: Annual Medical History & Permissions Form - Completed by Parent
Concordia requires parents to fill out an Annual Medical History & Permissions Form for each student before start of school each year. Prefer to receive the form before July 31.
